KOMPOSISI
Tiap 1 ml Otopain mengandung :

1. Polymyxin B sulfate 10.000 IU,
2. Neomycin sulfate 5 mg,
3. Fludrocortisone acetate 1 mg, dan
4. Lidocain HCl 40 mg

FARMAKOLOGI / CARA KERJA OBAT

Otopain tetes telinga merupakan kombinasi zat-zat yang efektif untuk mengobati bermacam-macam penyakit pada telinga.

Polymyxin B sulfate dan Neomycin sulfate merupakan antibiotik dengan spektrum luas, aktif terhadap berbagai bakteri : Pseudomonas aeruginosa, Staphylococcus aureus, Escheria coli, Klebstella, Enterobacter sp., Neisseria sp..

Fludrocortisone acetate mempunyai efek antiradang, antialergi, dan antipruritis.

Lidocaine hydrochloride merupakan anestesi lokal yang efektif untuk mengurangi rasa sakit pada infeksi telinga.

INDIKASIOtitis eksternal akut dan kronis disebabkan oleh bakteri yang peka terhadap Polymyxin dan Neomycin sulfate serta bila efek antiinflamasi dari kortikosteroid dan efek anestesi lokal diperlukan.

KONTRAINDIKASI

• Penderita-penderita yang hipersensitif terhadap salah satu komponen dalam obat Otopain.
• Jangan menggunakan Otopain, jika diduga atau diketahui adanya gangguan pada kanal bagian eksternal auditory yang disebabkan infeksi virus (sebagai contoh : virus Herpes simpleks atau virus Varicella zoster).

EFEKSAMPING:

1. Berupa reaksi hipersensitivitas jarang terjadi.
2. Neomycin sering menimbulkan kepekaan pada kulit. Ototoksisitas dan nefrotoksisitas juga pernah dilaporkan.
3. Efek samping dijumpai pada penggunaan topikal kombinasi antibiotik termasuk Neomycin dan Polymyxin B.
4. Berikut adalah efek samping lokal yang dilaporkan pada penggunaan kortikosteroid topikal, terutama yang dibalut dengan kuat, yaitu : rasa terbakar, gatal, iritasi, kering, folikulitis, hipertrichosis, muncul seperti jerawat, hipopigmentasi, dermatitis perioral, alergi kontak dermatitis, maserasi pada kulit, infeksi sekunder, atropi kulit, striae dan malaria.
5. Efek samping sistemik jarang dan dapat terjadi dari kadar plasma yang tinggi karena dosis berlebihan.

PERINGATAN/PERHATIAN :

1. Neomycin sulfate dapat menyebabkan sensitisasi kulit atau timbul iritasi.
2. Pengobatan tidak boleh lebih dari 10 hari karena risiko ototoksisitas lebih besar pada penggunaan jangka panjang. Penderita dalam pengobatan dengan tetes telinga yang mengandung Neomycin harus di bawah pengawasan klinik yang ketat.
3. Karena sifat asamnya yang dapat menyebabkan rasa terbakar dan menyengat, Otopain tetes telinga jangan digunakan pada penderita dengan perforasi membran tympani.
4. Jika infeksi tidak menunjukkan perbaikan dalam 1 minggu, uji kultur dan kepekaan harus diulangi untuk mengidentifikasi organisme penyebab dan menentukan apakah terapi harus diganti.
5. Tidak ada penelitian yang cukup memadai dan terkontrol dengan baik untuk penggunaan pada wanita hamil. Kortikosteroid hanya digunakan jika keuntungan yang didapat lebih besar dibanding risiko pada janin.
6. Ketika menggunakan produk yang mengandung Neomycin untuk mengontrol infeksi sekunder pada dermatosis kronis, seperti otitis eksternal kronis, pada keadaan ini kulit menjadi lebih peka dibandingkan kulit normal, terhadap berbagai bahan termasuk Neomycin.
7. Produk ini mengandung Sodium metabisulfite, sulfit dapat menyebabkan berbagai reaksi alergi, termasuk gejala-gejala anafilaksis dan yang mengancam jiwa atau episode asmatik yang kurang berat pada orang-orang yang peka.
8. Pengobatan jangka panjang dengan antibiotika ini dapat menyebabkan pertumbuhan yang berlebihan dari organisme yang tidak peka seperti jamur.

INTERAKSI OBAT
Neomycin sulfate bersifat bakteriostatik, menghambat aktivitas bakterisidal dari golongan penisilin dan derivat-derivatnya.

DOSIS DAN ATURAN PAKAI
Otopain diteteskan ke dalam lubang telinga dengan penetes. Sehari 2 – 4 kali 4 – 5 tetes, tergantung pada beratnya penyakit.

Lamanya pengobatan dengan produk ini seharusnya dibatasi sampai 10 hari berturut-turut.

PHARMACOLOGY / WAY OF ACTION
Otopain ear drops is a combination of substances that are effective for treating a variety of diseases of the ear. Polymyxin B sulfate and Neomycin sulfate is an antibiotic with broad-spectrum, active against a variety of bacteria: Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli, Klebstella, Enterobacter sp., Neisseria sp .. Fludrocortisone acetate has anti-inflammatory effects, allergy, and antipruritis.Lidocaine hydrochloride is an effective local anesthetic to reduce the pain of ear infections.

INDICATIONS 
Acute and chronic external otitis caused by bacteria that are sensitive to Neomycin sulfate and Polymyxin and when the antiinflammatory effects of corticosteroids and local anesthetic effects is needed. 

CONTRAINDICATIONS

• Sufferers who are hypersensitive to any of the components in the drug Otopain.
• Do not use Otopain, if it is suspected or known to a disturbance in the external auditory canal caused by viral infection (for example: Herpes simplex virus or varicella zoster virus).

Efeksamping: 

1. Rare form of hypersensitivity reaction.
2. Neomycin often cause sensitization of the skin. Ototoxicity and nephrotoxicity has also been reported.
3. Side effects observed in the use of topical antibiotic combinations including Neomycin and Polymyxin B.
4. Here are the local side effects were reported on the use of topical corticosteroids, especially wrapped with strong, namely: burning, itching, irritation, dryness, folliculitis, hipertrichosis, appearing like acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, maceration of the skin, secondary infection, skin atrophy, striae, and malaria.
5. Systemic side effects are rare and may occur from high plasma levels due to excessive dosage.

WARNING / CAUTION: 

1. Neomycin sulfate may cause skin sensitization or irritation occur.
2. Treatment should not be more than 10 days because of the risk of ototoxicity is greater in the long-term use. Patients in the treatment with ear drops containing Neomycin should be under strict clinical supervision.
3. Due to the nature of the acid can cause burning and stinging, Otopain ear drops should not be used in patients with tympanic membrane perforation.
4. If the infection does not improve within 1 week, culture and sensitivity test should be repeated to identify the causative organisms and to determine whether therapy should be changed.
5. No adequate studies and well-controlled for use in pregnant women. Corticosteroids are used only if the benefits outweigh risk to the fetus.
6. When using products containing Neomycin to control secondary infection in the chronic dermatoses, such as chronic external otitis, in this situation the skin to be more sensitive than normal skin, against a variety of materials including Neomycin.
7. This product contains sodium metabisulfite, sulfite can cause a variety of allergic reactions, including anaphylactic symptoms and life threatening or less severe asthmatic episodes in susceptible people.
8. Long-term treatment with these antibiotics can cause excessive growth of organisms such as fungi insensitive.

DRUG INTERACTIONS
Neomycin sulfate is bacteriostatic, inhibiting the bactericidal activity of the penicillins and its derivatives.

DOSAGE AND TERMS OF USE 
Otopain dripped into the ear canal with a dropper. A day 2-4 times 4-5 drops, depending on the severity of the disease. Duration of treatment with this product should be limited to 10 consecutive days.
